Dear sir/Madam,

1 am maintaining a savings account (number, 0000000000000) in your bank’s. (Branch name)

branch.

I lost a sum of 5000/rs when I was trying to withdraw money. from your bank's ATM at (location of ATM)

ATM code no000000000No cash came out of the ATM even after choosing the necessary options.

However, immediately 1 received an SMS regarding the debit/deduction of the amount from my

account. I waited for some time to see if the amount got reversed back to my account, but no SMS was

received. 1 even took out a mini statement from the ATM for my account, where I again checked on

the transaction. clearly, the debited amount was only reflecting but there was no reverse

Please do the needful as soon as possible to get back my money
